Wellness CoachUSD $50,000.00/Yr. to USD $55,000.00/Yr.Position OverviewSCOPE OF ROLE:The Wellness Coach will assist agency consumers with accessing needed community services (medical, psychological, substance abuse, vocational, food, clothing, etc.) through the development of a referral network, referral and follow-up.ESSENTIAL DUTIES & RESPONSIBILITIES:Provide case management to assigned caseloadDevelop and monitor individual service plans in conjunction with agency consumersAssist agency consumers with problem solving and crisis interventionAccompany agency consumers on medical, psychiatric, and public benefit appointments, as necessaryAct as advocate for consumer as appropriate; encourage and support self-advocacyIdentification of any problems in daily living skills; hands on assistance and/or referral in addressing problems in daily living skillsSupport Facilities Management staff in ensuring maintenance of apartment standards as outlined by SUS. Including funding sources i.e.; OMH, DOHMH, OASAS, HPD, HRASupportive counseling to agency consumers as neededRegular home visits and hospital visits, as necessaryMaintenance of a strong working relationship with all therapeutic support services to ensure the best quality of careDocumentation of all services in compliance with agency standardsAssist with moves with consumers and staff; also include lifting, packing and cleaningWork as a member of the social service team in the development of off-site recreation, socialization, educational and skill building activities with a view towards community inclusion and integrationLiaison to community-based services and groups including civic, social service government, and business organizationsParticipate in ongoing individual and group supervision, team meetings, case conferences, division meetings and trainingAdhere to on call cell phone system to facilitate crisis intervention, emergencies and other situations that require immediate actionOther duties as assignedJob RequirementsREQUIREMENTSBachelor's Degree requiredWorking knowledge of various computer programs including e-mail, Microsoft Word, Excel, PowerPoint and Windows Operating SystemPREFERRED QUALIFICATIONS & SKILLSTwo years' experience working with individuals with a history of homelessness, mental illness, substance use disorder and criminal justice involvement preferred.Experience in group work, tenant organization, case management, referral, and/or community outreach strongly preferredExperience or training in peer counseling or advocacy preferredBilingual English/Spanish speaking preferredCompany OverviewS:US IS AN EQUAL OPPORTUNITY EMPLOYERJoin a team of employees who cares about the wellbeing of others.
